Main Purpose of the Role

Responsibilities

Referral Coordination
📌 Ensure timely and smooth transfers for patients needing specialized care

📌 Maintain clear communication and follow-up with referral patients

Collaboration & Advocacy

Clinical & Logistical Support

💊 Collect and distribute medications and medical items
🧪 Facilitate lab/X-ray requests and retrieve results

Monitoring & Reporting
📊 Compile weekly, monthly, and quarterly referral reports
📊 Track and report bottlenecks with suggested improvements
📊 Ensure compliance with Infection Prevention & Control (IPC) protocols

Flexibility & Deployment

Requirements

Education
🎓 Nursing Degree or Diploma with nutrition training
